Notice of Privacy Practices

Privacy & HIPAA

1. Introduction & Commitment

Coranova DME ("Coranova," "we," "us," or "our") is committed to protecting the privacy of individuals whose information we handle in connection with our durable medical equipment (DME) services. This Privacy & HIPAA Notice is intended to explain, in clear and understandable terms, how we handle certain health-related and personal information, the rights that may apply to you, and how you can contact us with questions or concerns.

2. Use and Disclosure of PHI

When Coranova acts as a covered entity or business associate under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), we may use and disclose protected health information ("PHI") as permitted or required by applicable law. This may include using and disclosing PHI for treatment, payment, and healthcare operations, as well as for other purposes allowed or required by HIPAA, such as certain public health or legal obligations. We do not describe every possible use or disclosure in this Notice, but we strive to provide examples that help you understand the general ways PHI may be handled.

3. Individual Rights

Depending on your relationship with Coranova and the information involved, you may have certain rights under HIPAA or other applicable laws. These may include rights to request access to certain PHI, request amendments to PHI you believe is inaccurate, request restrictions on certain uses or disclosures, request confidential communications, or request an accounting of certain disclosures. The availability and scope of these rights can vary based on the circumstances and applicable law.

4. Our Legal Responsibilities

When HIPAA applies, Coranova is required to maintain the privacy and security of PHI as required by law, to provide individuals with certain notices about our privacy practices, and to follow the terms of our current Notice of Privacy Practices where applicable. We also strive to comply with other privacy and security obligations that may apply to non-PHI information, such as certain personal information collected through our website or services.

5. Safeguards

Coranova uses reasonable safeguards designed to protect information we handle from unauthorized access, use, or disclosure. These safeguards may include appropriate administrative, physical, and technical measures based on the type of information handled and the requirements applicable to our organization. We do not list specific security technologies here, as those details may change over time and may depend on the systems and services we use.

6. Disclosures Required By Law

In some circumstances, we may be required by law to use or disclose certain information, including PHI. For example, we may be required to disclose information in response to a valid court order or other legal process, to comply with certain government reporting obligations, or as otherwise required by applicable law. When we are required to make such disclosures, we will limit the information disclosed to what is reasonably necessary to meet the legal requirement.

7. Questions or Privacy Complaints

If you have questions about this Notice or believe your privacy rights may have been violated, you may contact Coranova using the information below. You may also have the right to file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. 8. California Privacy Rights

Because Coranova serves patients and customers in the United States, additional privacy rights may apply depending on your location, the information involved, and applicable law. California residents may have additional rights under applicable California privacy laws regarding certain personal information, including rights relating to access, deletion, correction, and limitations on certain uses or disclosures of personal information. Any California-specific rights and procedures will be provided where applicable.
